Medicare Dashboard Helps Providers Manage Multiple Chronic Conditions
April 3—A new online data tool gives healthcare industry professionals access to current data that can help them better coordinate care for patients with multiple chronic conditions, according to the Centers for Medicare & Medicaid Services (CMS).
The Medicare Chronic Conditions Dashboard provides researchers, physicians, public health professionals, and policymakers with data on where multiple chronic conditions occur, which services they require, and how much Medicare spends helping beneficiaries with multiple chronic conditions.
The dashboard is part of the U.S. Department of Health and Human Services’ (HHS) Initiative on Multiple Chronic Conditions, established in 2009. More than two-thirds of Medicare beneficiaries have multiple chronic diseases such as heart disease and diabetes, and that number will rise with an aging population, according to a release.
