Summary
Preventing readmissions has been a longtime quality-improvement priority for many hospitals and health networks. Now it has become a financial imperative too, thanks to provisions of the Patient Protection and Affordable Care Act which set penalties for hospitals with higher-than-average rates of avoidable readmission for patients with specified conditions. Because the financial impact to hospitals can be substantial, organizations must be able to measure the frequency of and reasons for hospital readmission and quantify the margin impact, as well as identify key indicators that put patients at risk. This requires access to new sources of data, new metrics that cross the continuum, and business logic that can predict variance and model best practices.
