Certification - Additional Specialty Application

This application is intended for certified members in good standing who have earned an additional specialty by passing the certification exam for the specialty. To receive a certificate, the application must be submitted with the processing fee. Allow 60-90 days for processing. Certificates are issued to the Chapter President for distribution. They are not mailed directly to the member.
