Partners HealthCare in Boston, Mass., has implemented an organization-wide population health management strategy to improve the health of its patients while reducing the overall cost of care. In this session, Ryan Thompson, MD, MPH, Director of the Continuous Care Initiative of the Massachusetts General Physicians Organization and Associate Medical Director of the Massachusetts General Hospital Integrated Care Management Program for high-cost Medicare patients, will discuss how Partners has used its population health management strategy to operationalize clinical transformation system-wide, addressing both the challenges faced in implementing the strategy and the strategy’s successes to date.
