An online survey conducted by HFMA on behalf of the chapter every November. The chapter receives this summary and replies remain anonymous. This is a very useful tool for the leadership as it assists them in meeting the needs of the members.
Members Award Program
Arizona Chapter is augmenting the Awards program with three new prestigious awards in recognition of members that have made substantial contributions to the Arizona Chapter of HFMA and the healthcare financial management profession. These recognitions will be awarded annually. Members will be recognized based on nominations from other HFMA members.
All nominations will be reviewed by the Awards Committee and recommendations will be presented to Board of Directors for approval. The selected members will be honored at the Spring Conference each year and presented with an engraved glass plaque award.
Criteria for the awards:
- The recipient must be an HFMA member.
- Both Providers and Vendors will be recognized.
- Awards will be given annually.
- Three recipients will be awarded annually (at least two providers and at least one vendor)

The professional selected for this category will be an individual that has made a substantial contribution to HFMA – Arizona Chapter as demonstrated by 3 years of service since the Muncie Gold Award, significant service at the Chapter level, and activily participating in chapter activites (planning, education, leadership).
