Yale New Haven Hospital asks the following housing-related questions to determine whether patients are homeless. - Where have you been living during the past two months?
- Is this reliable housing that you own, rent, or stay in as part of a household?
- Are you able to return and stay there following discharge?
If yes, are you able to receive a visiting nurse there?
(If no, discharge planner should consider whether medical respite is needed post-discharge.)
