Midyear coverage changes: 6 things to know
The biggest enrollment change happened in the ACA and off-exchange individual market.
Enrollment is increasing employer plans and Medicare Advantage (MA), while individual, Medicaid and fully insured plans lost enrollees.
A recent Mark Farrah Associates brief assessed year-over-year enrollment trends, comparing membership changes from the second quarter of 2025 to the second quarter of 2026. Here are its leading findings as of June 30.

- MA enrollment increased by 700,000 more seniors to reach 35.1 million enrollees.
- The self-funded employer group segment gained slightly more than 1.1 million members to reach 135.8 million enrollees.
- The individual market for ACA marketplace and off-exchange coverage lost more than 4.5 million enrollees, falling from 26.5 million members to nearly 22 million.
- The employer-group risk segment, which includes Federal Employees Health Benefit Plans, lost 2.1 million members to reach about 46.7 million enrollees.
- Both managed Medicaid and fee-for-service Medicaid lost almost 4.1 million combined members to reach 73 million beneficiaries.
