2026 Physician Fee Schedule Specialty Impact Tables

The specialty impact analyses are contained in one spreadsheet that includes information on almost all of the designated Medicare specialties (over 100 specialties) and the associated detail. This includes, for example, cardiology, emergency medicine, family practice, general surgery, internal medicine, radiology, radiation oncology, podiatry, and rheumatology. The user chooses the specialty of interest, such as family practice in a dropdown box, and the spreadsheet displays the overall impact for that specialty and detail code level information on the top 250 codes as ranked by estimated 2026 total payment. There is also a separate spreadsheet tab that shows the overall estimated impacts by specialty – similar to the regulatory impact table in the Medicare Physician Fee Schedule (PFS) final rule.

Analytic Note: The estimated specialty impacts vary from CMS’ regulatory impact table (Table D-B7 in the final rule) as the estimates include the 0.25% APM Non-Qualifying PFS Update and the 2.5% single year statutory adjustment. There is an increase of almost 3.25 percent to the qualifying APM PFS CF and 2.75 percent to the non-qualifying APM PFS CF based on the statutory updates. For the specialty impact analyses, the non-qualifying APM CF is used for the 2026 payment calculations.
