DMEPOS Fee Schedule Rate Comparison Tables for 2025 and 2026

This spreadsheet compares the calendar year (CY) 2026 DMEPOS fee schedule to the 2025 DMEPOS fee schedule. CMS released the CY 2026 update to the DMEPOS fee schedule on December 19, 2025, per its usual approach, through a change request transmittal and MLN Matters. The transmittal (r13519cp.pdf) and MLN Matters article (MM14326 – DMEPOS Fee Schedule: CY 2026 Update) can be found at CMS’ website. For CY 2026, per statutory guidance, CMS applies a fee schedule update factor of 2.0 percent to most DMEPOS items, 2.8 percent for items provided in former competitive bidding areas and for mail order diabetic testing supplies, and 2.7 percent for lymphedema compression treatment items.

The DMEPOS update factor, per statute, is based on the CPI-U for the 12-month period ending June 30, adjusted by the change in the economy-wide productivity (i.e., referred to as the Total-Factor Productivity). The DMEPOS 2025 update is 2.0 percent – the CPI-U increase of 2.7 percent reduced by the 0.7 percent increase in the productivity adjustment. The update factor for items provided in former competitive bidding areas, mail order diabetic supplies and lymphedema compression treatment items are based on 12-month period CPI-U calculation without a productivity adjustment.

The 2026 DMEPOS spreadsheet contains 6 tabs. Cells are locked for the first four tabs but content can be printed or copied and pasted into another spreadsheet for manipulation. Within the locked spreadsheets, users can filter but not sort. The tabs are as follows:

  1. DMEPOS Payment Comparison: Compares 2025 and 2026 payment rates for DMEPOS items and services in HCPCS order. Includes an indicator for items whose payment are adjusted based on information from competitive bidding. These items include separate rates for rural areas. User can select geographic area in cell “C1”. National rates are calculated based on the contiguous United States and exclude Puerto Rico, Virgin Islands, Hawaii, and Alaska. Rural cells with $0.00 indicate that the rural rate is not applicable and the nonrural rate applies.
  2. DMEPOS HCPCS Lookup: Allows user to look up descriptive and payment information on a given HCPCS and geographic area for DMEPOS items. User chooses HCPCS in formula bar in cell “B2” and geographic area in cell “B3”. Spreadsheet automatically displays relevant information for the HCPCS including payment rates, as applicable, based on the relevant modifier such as purchased (new or used), or rented.  Modifier and category definitions are included on this sheet.
  3. Multiple HCPCS Rate Lookup:Allows user to look up payment rate information for a given HCPCS (and its specific modifier) and displays how the rate varies, as relevant, by geographic area and by rural/non-rural. Rural cells with $0.00 indicate that rural rate is not applicable and the nonrural rate applies.
  4. Cap Rental Calculator: Displays the monthly capped rental amount, and the total rental payment over 13 months for HCPCS codes with a category indicator of “CR” or capped rental.
  5. National Mail-Order CBA: Reproduces the 2026 DMEPOS rates for mail order diabetic testing supplies such as blood glucose/reagent strips and lancet devices.
  6. 2026 Fee Schedule (CBAs): Displays the fee schedule for items and services provided in a former CBA based on the single payment amounts adjusted by the inflationary adjustment for 2026.
