FY 2027 Final IPPS MS-DRG Rate Comparison to FY 2026 Rates

This spreadsheet compares the FY 2027 IPPS final rule relative weights and standardized amounts effective 10-1-26 with the final FY 2026 weights and rates. The spreadsheet has five tabs, as follows:

• Sort by MS-DRG 2027: This tab compares the relative weights and other elements in Table 5 of the FY 2027 final rule with Table 5 of the FY 2026 final rule. Payment rates were calculated using the final FY 2027 standardized amounts effective 10-1-26 and the FY 2026 standardized amounts. The data are presented in MS-DRG order in this tab.

Note: CMS makes changes to the existing 772 MS-DRGs, effective for FY 2027 that results in 768 MS-DRGs by adding 14 MS-DRGs and deleting 18 MS-DRGs. These changes are described in detail in the summary of the 2027 IPPS final rule.

• Sort by % Change 2027: Same information as the previous tab but sorted by percent change in the relative weights.
• Rev MS-DRGs 2027: This tab lists the MS-DRGs affected by FY 2027 final rule revisions and compares discharges using the final rule 2027 Version 44 Grouper to the Version 43 Grouper. Sorted by the difference in number of discharges. The number of discharges was extracted from the AOR/BOR files.
• MS_DRG Changes: This tab lists the new MS-DRGs for FY 2027, the deleted MS-DRGs, and changes to DRG titles. These changes are described in detail in the summary of the 2027 IPPS final rule.
• FY 2027 final rule standardized amounts: This tab is a copy of Tables 1A to 1D published on the CMS website as part of the final rule.
